Support after leaving hospital following a brain injury
Leaving hospital after a brain injury can feel overwhelming. You may have questions, feel unsure about what happens next, or simply want someone to talk to.
Our ABI Navigators are trained volunteers who offer friendly, practical support to people after they leave hospital following an acquired brain injury (ABI).
They are there to listen, offer encouragement and help you find the information and support you may need.
How can an ABI Navigator help?

What does an ABI Navigator do?
Your ABI Navigator is there to support you as you adjust to life after hospital.
They can:
- Listen to you and talk about how things are going.
- Offer encouragement and reassurance.
- Help you find information about brain injury.
- Help you understand what support is available.
- Help you connect with local services and community support.
- Be there as you take your next steps after leaving hospital.
You can talk to your Navigator about what matters to you. There is no pressure to do everything at once.
Who can use the service?
The ABI Navigator service is for people who have recently left hospital following an acquired brain injury.
An acquired brain injury can happen because of a stroke, accident, illness, infection or other cause.
You do not have to know exactly what support you need. We can talk to you about your situation and help you understand what may be available.
What happens when I have an ABI Navigator?
1. You will be introduced to your Navigator
We will introduce you to your ABI Navigator and explain how the service works.
2. You can talk about what you need
Your Navigator will listen and give you the time and space to talk about how things are going.
3. Find support that works for you
Your Navigator can help you find useful information, services and support in your local community.
You don’t have to do it alone
Life after brain injury can take time to adjust to.
You may feel tired, confused, worried or unsure about what happens next. Things that used to feel easy may suddenly feel more difficult.
Having someone to talk to can make a real difference.
Your ABI Navigator is there to listen, support you and help you find your next step.
